Opportunities in the Medical Home
The medical home health care model is going mainstream. What has been a successful model of care delivery within pediatric Medicaid populations is gaining interest with commercial health plans and employer groups. Driving some of the interest is the increasing acknowledgment that a lack of care coordination within the U.S. health care system is perpetuating fragmentation of care, rising costs and poor health outcomes.
Casenet’s Director of Clinical Solutions, Jo Anne Hunt, ARNP, CCM, looks at the shift in focus toward medical home health care models in Managed Care Outlook.

Casenet Unveils TruCare 4.0-New Care Management Platform Delivers Personalized, Member-centric Care
BEDFORD, Mass. and SAN DIEGO, Calif. – June 3, 2009 – Casenet, Inc., a provider of innovative care management software solutions, today unveiled TruCare™ 4.0, its next generation care management software suite. TruCare 4.0 builds on a foundation of six years of exclusive care management software development specifically for case, disease and utilization management. It enables care delivery organizations to utilize their proprietary clinical intelligence and best practices to drive individualized care based on the unique characteristics of the different member populations they serve.
TruCare 4.0 brings to market a number of innovative features not available elsewhere that enhance care and streamline workflow for care delivery organizations. These include:
- Differential Care™— This innovative functional architecture provides care delivery organizations, including public and private payers and wellness companies, a fully integrated solution with easy to use tools that can tailor system behavior to the needs of each individual population, such as by region, plan design or health condition. Knowledge collected about each member enables personalized interactions and content in the member portal, for greater member empowerment and partnership in wellness activities.
- TruFlex Points™—With this new way to configure a care management system, users can set up what they need for their business today and add additional configuration or complexity over time. Users are able to tailor the system to their choosing (i.e. per region, per employer group, per population, etc.) and make changes and updates anytime. It collapses the typical amount of effort and resources needed to configure and implement care management solutions, allowing health management organizations to have a greater business impact for a fraction of the effort.
- TruMobile™—A mobile capability that offers “always-on” availability providing users full application functionality anywhere, anytime even without an Internet connection. This is especially useful for staff working in locations where an Internet connection is not possible, such as hospital intensive care units or employer worksites.
“With TruCare 4.0, Casenet is revolutionizing care management by enabling clients to provide high-quality, member-centric care using innovative tools and functionality. We help users manage their members, not their care management system,” said Kevin Brown, CEO, Casenet. “Through our TruFlex Points concept in particular, clinical teams will have the tools in-hand to allocate appropriate resources and easily incorporate regulatory, organizational and evidence-based guidelines, all while configuring the system in the way that works best for them.”
Member-centric, rather than case-centric, solutions like TruCare 4.0 are becoming increasingly valuable to care management organizations. In addition, many care management solutions available today fail to combine case and disease management tools in an easy-to-use, integrated platform, or enable multi-disciplinary teams to simultaneously collaborate, coordinate and manage personalized care for members. This is where Casenet’s TruCare 4.0 stands out, setting a new standard of excellence for the industry.
Note to editors: Casenet is previewing TruCare 4.0 at AHIP Institute 2009, booth #219, in San Diego June 3-5.

Use predictive modeling in Medicare populations
Predictive modeling can help identify beneficiaries with the highest risk and capture those individuals with emerging risks that otherwise might missed. Coupling predictive modeling with integrated care management and appropriate technology can help case workers ensure Medicare beneficiaries receive efficient, high quality, cost effective healthcare.
Why use predictive modeling? Programs working with Medicare beneficiaries are being challenged to go beyond traditional utilization, case and disease management services with limited clinical resources. To meet that challenge, programs need innovative ways to identify beneficiaries in the population not only with costly chronic conditions but also those at the greatest risk for adverse outcomes.
Jo Anne Hunt, ARNP, CCM and Casenet's director of clinical solutions, talks about how integrated predictive modeling tools in a care management system can help users analyze data using complex logic and rules for millions of beneficiaries in a short period of time.

Considerations for Outcomes Reporting
As the economy tightens, clients are asking for verification that the dollars they spend on healthcare activities and programs are, in fact, producing measurable outcomes.
Key to an organization’s ability to analyze and report an outcome is collection of the appropriate data. Proactive identification of metrics to be captured, measured, and reported on is necessary for meaningful outcomes reporting, which Casenet’s Director of Clinical Solutions, Jo Anne Hunt, ARNP, CCM, discusses in the March edition of Managed Care Outlook.

Making the Leap to Paperless Case Management
The decision by an organization to move from a paper-based case management system to a paperless one is only a first step in developing a successful strategy for integration of the technology into the actual case management processes.
Case management programs that effectively manage the process can realize increased productivity, better client outcomes and higher adoption rates by their case managers in accepting the change.
Casenet’s Director of Clinical Solutions, Jo Anne Hunt, ARNP, CCM, looks at how to manage the transition and potential pitfalls of moving to paperless case management in Advance for Health Information Executive.

Cancer top killer by 2010
Cancer will overtake heart disease as the world’s top killer by 2010, which translates into double global cancer cases and deaths by 2030, according to a report by the World Health Organization. Rising tobacco use in developing countries is believed to be a big reason for this.
In Managed Healthcare Executive Lori Donovan, Casenet’s director of clinical operations, comments on how cancer-specific patient incentive programs can help motivate positive change and the maintenance of healthier lifestyles for those dealing with this disease in Tracey Walker's article "Cancer top killer by 2010".

Integration of Predictive Modeling Can Drive Better Care in Medicare Populations
Medicare is the constant victim of federal budget cuts. Those working with Medicare beneficiaries are challenged to find ways of providing expanded services while containing costs.Adding to the complexity is the high prevelance of costly chronic conditions in the Medicare population. Not all individuals with a chronic condition have the same overall healthcare needs, so a cookie cutter approach to resource allocation and program interventions may lead to missed opportunities for impact and produce less than optimal results.
Casenet Names Kevin Brown Chief Executive Officer
BEDFORD, Mass. – Casenet, Inc., a provider of innovative care management software solutions, today announced that Kevin Brown has been appointed chief executive officer effective immediately. Brown has more than twenty years of experience managing fast-growth technology companies within the healthcare industry. His expertise in enterprise product development and customer support across payer markets will add further focus to Casenet’s mission of creating cutting-edge care management software solutions backed by solid customer service support. Former CEO Brian O’Neill will be transitioning to an advisory role.
“As CEO, Brian has successfully led Casenet’s rapid growth as an innovator in care management software from its early stages. The board thanks him for his considerable contributions to Casenet and wishes him every success in the future,” stated Peter Grua, partner at HLM Venture Partners.
Mr. Grua continued, “We are excited to have Kevin take the helm at Casenet. Kevin is an experienced entrepreneur with a successful track record of growing enterprise solution healthcare companies and his expansive network and deep industry knowledge will provide the team with an even better understanding of where the market is going, which will drive innovation and help anticipate and fulfill clients’ needs.”
Brown was formerly president, CEO and chairman of the board of AMISYS Managed Care Systems, a provider of health insurance and managed care information systems. During his ten year tenure, he grew the company from a one million dollar managed healthcare software business to a 68 million dollar (1998 revenues) publicly-held market leader with 105 health plan customers. (ASI) where he increased revenues by 20 percent, developed a consumer-directed processing product, and drove a customer service initiative that resulted in the retention of more than 97 percent of customers. Additionally, Brown completed an acquisition of a profitable offshore business that helped ASI realize significant cost savings and improved ASI’s market position. ASI was acquired by DST Systems (NYSE: DST) in November 2006 forming DST Health Solutions (DSTHS) when ASI was merged with DST’s existing healthcare business. Brown served as chairman of DSTHS since its formation, which serves a customer base of more than 150 payers.
“Casenet has impressed me by entering a crowded market and really carving out a niche for itself through offering health plans, governments and wellness organizations an innovative software platform that meets their needs from both an architecture and usability perspective,” said Kevin Brown, CEO of Casenet. “Brian has done a remarkable job of growing Casenet from an idea to a competitive player in the industry. I look forward to expanding the Casenet market opportunity as well as working with our expanding roster of clients to ensure we are taking the product where they need it to go.”
Brown serves as a director on the board of CareSource Management Company, which oversees the CareSource businesses, including CareSource Ohio, one of the largest independent Medicaid HMOs in the United States. He also serves as chairman of the board of CareSource Michigan, a Medicaid HMO that covers more than 50,000 insured lives. Brown is a graduate of City College of New York.

Opportunities in the Medical Home
The medical home healthcare model is going mainstream. Commercial health plans and employer groups are continuing to show interest in the potential value of the medical home approach. Driving some of the interest is the increasing prevalence and associated costs of chronic conditions in the United States coupled with the growing shortage of primary care clinicians. Primary care providers, health plans, and disease management companies all focus benefits and services on improving health outcomes while trying to contain the cost of care provided.
Wellness Program Outcomes
Organizations can measure ROI related to wellness programs, but challenges persist.
Organizations of all sizes are incorporating wellness programs into their health care cost containment strategies. Driving some of this interest in wellness initiatives is the realization that employee lifestyles account for a vast majority of an employer's health care costs and that the improved overall health of employees has been shown to contribute to long-term health cost avoidance and increased productivity. For example, the American Institute for Preventive Medicine reviewed published studies on wellness program outcomes and found $3.48 in reduced medical cost for every dollar spent. This type of objective return on investment (ROI) is key to an organization's decision to continue to offer wellness programs.
Commonwealth Care Alliance Selects Casenet to Unify Patient Information and Improve Care for Members
Case Management Solution Will Help Commonwealth Care Alliance Coordinate Care and Improve Workflow
BEDFORD, Mass. – October 8, 2008 – Casenet, Inc., a provider of innovative care management software solutions, today announced that Commonwealth Care Alliance, a not-for-profit care delivery system, will employ Casenet’s case management solution to more effectively manage its members’ care. The Casenet platform fosters collaboration and communication between direct care providers and Commonwealth Care Alliance. In doing so, Commonwealth Care Alliance fulfills its promise to provide responsive, continuous and cost-effective medical and social services for people with complex medical needs.
“The Casenet platform and philosophy are a perfect fit for our team-based approach to care management. Casenet is designed to unify multiple systems and provides everyone involved with a complete, single view of each patient’s record,” said Lois Simon, COO, Commonwealth Care Alliance. “With Casenet, we’ll be able to improve the quality and delivery of care while improving efficiencies by streamlining workflows. This is critical as our members have complex medical needs and our care managers have demanding jobs to do in meeting the needs of our members.”
As a consumer-governed organization, Commonwealth Care Alliance’s mission is to provide the best possible care tailored to each individual’s needs. Prior to Casenet, Commonwealth Care Alliance used different applications for their varied activities associated with managing membership in the organization. Now, Casenet will provide Commonwealth Care Alliance member services staff, enrollment staff and care managers as well as the provider network with a single data source for thorough and up-to-date patient information to aid in more accurate and efficient decision-making. By integrating the claims payment system and replacing enrollment and care management systems with Casenet, Commonwealth Care Alliance will benefit from a centralized, cross-disciplinary design that increases efficiencies and collaboration, thereby improving care recommendations and overall health outcomes, and better managing healthcare costs.
“Commonwealth Care Alliance is a clear leader in care delivery and has the proven ability to achieve successful outcomes for patients through an integrated healthcare model,” said Bret Siarkowski, founder and CTO, Casenet. “The Casenet case management solution will enable Commonwealth Care Alliance to take collaborative care to the next level, continuing its commitment to providing members with the best possible care.”
Commonwealth Care Alliance worked with The Gugenheim Group, LLC (http://www.thegugenheimgroup.com), who led the software search and selection process. Together, they will implement requirements for delivering an integrated solution, including medical management, enrollment and eligibility, single provider platform, and tools to enable and support the delivery of highly collaborative care.
“The process of defining and selecting a care management vendor made it clear that the solution needed to enable true team collaboration focused on the member, rather than the software,” said Mary Gugenheim, project lead, The Gugenheim Group. “After evaluating a number of solutions, Casenet was the only one that provided that flexibility and focus.”
About Commonwealth Care Alliance
Commonwealth Care Alliance is a not-for-profit care delivery system committed to the provision of integrated health care and related social support services. Created in 2003, Commonwealth Care Alliance is a “consumer-governed” organization offering a full spectrum of medical and social services for people with complex needs covered under Medicaid and for those “dually eligible” covered by both Medicaid and Medicare. A case for case management
Government Health IT talks about how Pennsylvania, a Casenet customer, is using information technology to tame costs and improve health care for Medicaid patients.
Pennsylvania’s Department of Public Welfare offers case management and disease management as part of its Access Plus initiative for participants in the state’s medical-assistance programs. The department has won praise for incorporating population health and disease management into a system that was once bogged down in cumbersome, manual processes.
Recently, Harvard University’s Ash Institute for Democratic Governance and Innovation ranked Access Plus among the top 50 programs in its 2008 Innovations in American Government Awards.
Harvard cited the program for taking “the incentives for prevention and disease management found in managed care and [adapting] them to fee-for-service settings.”
Casenet Introduces New Version of Integrated Care Management Platform
Casenet Version 3.0 Adds Enhanced Reporting and Always-On Availability to Easy-to-Use, Extensible Solution
BEDFORD, Mass. – July 29, 2008 – Casenet, Inc., a provider of innovative care management solutions, today announced the release of the newest version of its care management software. Version 3.0 offers fully integrated population management tools designed for use by public and private payers and wellness companies. Casenet’s platform is a member-focused solution that provides easy-to-use reporting tools and “always-on” availability while offering true collaborative care by giving multi-disciplinary teams the capability to consolidate member information into a single holistic view.
Member-centric, rather than case-centric, solutions are becoming an increasingly valuable tool used by care management organizations. Many care management solutions on the market today do not offer their case and disease management tools in one integrated platform. Member-centric solutions enable multi-disciplinary teams to simultaneously collaborate, coordinate and manage personalized care for members.
“Most healthcare insurers face a host of challenges with their care management system, including inflexible technology and fragmented, disconnected legacy systems that don’t support the sharing of information,” said Joanne Galimi, research vice president, Gartner. “The need to share care management information in real time among stakeholders and various applications in a health insurer organization is increasingly a key differentiator. Health insurers must adopt care management applications that are able to interoperate within and outside of the payer organization to be able to meet both current and future IT requirements.”
Casenet version 3.0 incorporates a number of innovative features to its integrated care management platform that will enhance care and streamline workflow, specifically:
Graphically-based Reporting—The easy-to-use “point, click, done” report functionality provided in Casenet version 3.0 offers users a consistent view of information and report generation across multi-disciplinary teams. Staff can quickly identify key pieces of information on individuals or a population of members without searching in disparate information systems. User-friendly filtering tools allow users to efficiently sift through large datasets pinpointing key data points/events. Graphical representation of selected data is only one click away. For example, review of information related to appeals and grievances, regardless of the program from which the request was generated, can be reviewed for organizational compliance with state and federal regulations. Report information produced is complete and actionable, facilitating not only organizational reporting but also related quality improvement initiatives.
Member-focused—Casenet offers a fully integrated care management, disease management and utilization management platform enabling users to work on the same case at the same time. Because version 3.0 is focused on the member or patient, not the case itself, caregivers have the ability to access and update information on multiple cases at the same time. This allows for a unique type of collaboration focused on the individual along the entire care continuum. For example, a disease manager can be in a member’s record updating a care plan at the same time as the social worker is documenting information while in the member’s home and a utilization review nurse is approving outpatient use of durable medical equipment.
Always available—Version 3.0 enhances Casenet’s TruMobil™ solution that offers “always-on” availability providing users full application functionality anywhere, anytime even without an Internet connection. Entries are auto-synchronized and updated when the user reconnects. This is especially useful for staff working in locations where an Internet connection is not possible, such as hospital intensive care units, employer worksite clinics or a member’s home.
“Casenet is dedicated to helping our clients streamline their workflow and provide high-quality member care. We have already seen our platform make a difference for public and private payers and their members,” said Bret Siarkowski, founder and CTO, Casenet. “Now with version 3.0, we are making it even easier for care managers to access member information whenever and wherever they need it, and work with data to help evaluate program effectiveness and report on outcomes and ROI.”

Commonwealth of Pennsylvania Successfully Deploys Casenet Care Management System
Waltham, Mass. – April 16, 2007 – Casenet, Inc., a provider of innovative enterprise care management solutions, today announced that the Commonwealth of Pennsylvania Department of Public Welfare (DPW) successfully deployed the Casenet care management solution as a component of its Medical Management Review System (MMRS) announced in January of 2007 by Affiliated Computer Services, Inc., (NYSE: ACS). Casenet and ACS have partnered to deliver a comprehensive MMRS solution for the Department that includes prior authorization and medical review; case management and utilization review; and issues tracking and resolution management.
The Casenet system is currently in production use for high risk obstetrics, neonatal intensive care unit, wound management, pain management, prosthetics and general case management populations. The Department chose Casenet’s flexible care management solution as a component of the MMRS to coordinate enhanced healthcare services for its recipients while emphasizing quality and effectiveness. The Casenet solution will integrate with the ACS SmartPA system to assist DPW in meeting their commitment to provide responsive, timely and the highest level of service quality while managing increasing health care costs. Using Casenet, the Department will be able to collaborate, coordinate and manage personalized health care plans for each of their recipients.
According to Kathy Willis, the Medical Assistance Fee-For-Service Program Director for the Department of Public Welfare, “The DPW team has interacted with Casenet throughout the implementation process as they configured a solution that automates our processes and meets our specific needs. Together we were able to meet our delivery milestones and, because the system aligned to our processes, the users went live without experiencing the traditional challenges associated with new system ramp up.”
“We are very excited about the potential opportunities the Casenet solution provides our organization as we look to implement new care and quality programs,” added Lori Donovan, Lead Clinical Consultant for Medical Review/Medical Project Management for the Department of Public Welfare. “Leveraging this solution will help us to streamline our case and utilization management processes. It also has one of the most impressive reporting tools I’ve seen that enables non-technical users to create custom reports giving us greater real-time visibility into our operations and outcomes and allowing us to make better decisions faster.”
Following a smooth transition to production, DPW is now using Casenet to successfully complete their core care management activities. “I’ve used a lot of applications but this is one of the most intuitive and easy-to-use systems I’ve experienced,” added Jean Whitehead, Lead Clinical Consultant for Intensive Medical Case Management for the Department of Public Welfare. “Our users have become productive with the software very quickly and we are already experiencing significant process improvements through the integrated nature of Casenet’s comprehensive care management system.”
“We were impressed how the Department was able to bring together a diverse group of external vendors and internal teams to successfully meet an aggressive deployment schedule,” commented Tony Larson, Vice President of Marketing and Business Development with Casenet. “The Department is committed to state-of-the-art practices relative to the delivery of healthcare services. We look forward to continuing to help them achieve this goal and meet the constantly changing healthcare needs of their recipients.”
