Home and Community-Based Services Enable Members To Live At Home Cost-Effectively
Healthcare is rapidly evolving to meet the needs of an aging population who experience multiple co-morbid conditions and are living longer. Enhanced coordination of care, including home and community-based services (HCBS), is essential to reducing medical costs and delivering improved population health.
It is estimated that “the Medicaid program can provide HCBS to three people for the cost of serving one person in a nursing home."1 As a result, home and community-based services are being utilized more heavily in traditional treatment plans to enable the management of members outside a conventional healthcare facility while proactively managing chronic conditions. When the appropriate use of home and community-based services are incorporated into a member’s care, the plan’s members are able to stay in a more familiar, independent setting and take a more proactive role in their care while allowing treatment to be administered in a cost effective manner.
The Casenet TruCare Home and Community Services (HCS) solution facilitates coordination of home and community-based services for long-term care and waiver programs including those of Medicaid and other government-sponsored programs. TruCare HCS facilitates better team-based care coordination by enabling care team members to access both clinical and ancillary service requests directly from the member care plan including the creation, authorization and costing of these services.
